JOB PURPOSE:
Conduct initial eligibility screening for the Homelessness Prevention Program designed to support
households at imminent risk of homelessness. The program provides case management
and flexible emergency assistance aimed at stabilizing housing, preventing entry into
unsheltered homelessness, and addressing the factors contributing to housing instability.
he program will have direct referral pathways from key partners within school districts as
well as several Justice of the Peace courts throughout Houston, Harris County,
Montgomery County, and Fort Bend County. These partnerships are intended to ensure
timely identification and support for households experiencing imminent housing crises.
This is a grant-funded program.

ESSENTIAL JOB RESPONSIBILITIES:
� Develops effective, trusting relationships with program participants by utilizing trauma-
informed care and a strength-based approach. Models positive interactions, verbal
guidance, and effective communication.
� Conducts initial eligibility screening for individuals seeking program assistance. Advocates for
eligible participants and provides general information and community referrals for those not
meeting program eligibility requirements. Completes Intake for eligible participants by
evaluating individual and/or family needs. Sets up initial client files for clients being assigned
to a Service Coordinator.
� Answers incoming calls, and emails, and assists with office management duties.
� Documents case notes daily, submits comprehensive participant files, completes reports as
needed, and enters documentation into the Salesforce system. Prepares necessary incident
reports.
� Maintains ongoing communication with multi-disciplinary teams across the program to assist
participants in obtaining and maintaining services to meet needs and promote stability.
� Other duties as assigned.
� Demonstrate Exceptional customer service, in Everything you do, by placing the child,
family, Veteran or client first to support our mission to "Empower people to build better
lives for themselves, their families, and their communities."
ESSENTIAL QUALIFICATIONS:
EDUCATION: Bachelors degree preferred

EXPERIENCE: 2+ years of experience in a social service agency, 2+ years in a customer
service-focused environment. Experience with mental health and/or a homeless population is a
plus.
ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in
the employer's sole discretion.
LICENSES: Driver's License with clear record.
VEHICLE: Must have daily use of a vehicle without prior notice.
OTHER: Must be available and willing to travel to various locations and with such frequency as the
business need dictates.
