We offer an alternative to nursing homes or assisted living facilities. Our Case Managers coordinate and manage care providers and other daily living activities, ensuring that services align with clients' needs and meet their expectations.

Position Summary
We are seeking a compassionate and dedicated Case Manager to join our team. The ideal candidate will enhance the quality of care, promote client satisfaction, and support cost-effective solutions. This role is responsible for the full cycle of patient management, including assessment, planning, implementation, monitoring, and evaluation of services.

Key Responsibilities

  • Bachelor's degree in Human Services or a related field is preferred.
  • Coordinate and deliver client-centered care that is safe, timely, efficient, and equitable.
  • Familiarity with how ADLs and IADLs impact an individual's level of care.
  • Conduct monthly check-ins, develop service plans, and manage all phases of the case lifecycle.
  • Support clients in maintaining independence within their community through resource linkage and advocacy.
  • Attend monthly mandatory meetings at the Area Office.
  • Maintain reliable internet access and a mobile hotspot.
  • Engage regularly with clients to monitor satisfaction and track progress.
  • Document all case activity accurately and in compliance with SCDHHS guidelines.
  • Promote high-quality outcomes with the involvement of the client and their support network.
  • Meet all monthly deadlines and reporting requirements.
  • Attend monthly staff meetings and comply with agency policies and procedures.
  • Experience in addressing psychological and emotional aspects of care.
  • Strong communication, organizational, and time-management skills.
  • Effective problem-solving and multitasking abilities.
  • Compassionate team player with a commitment to client advocacy.

Mandatory Requirements

  • Minimum of two years' experience in case management, including planning and assessment.
  • Must reside in or be able to serve the Conway regional area counties: Georgetown, Horry, Marlon, and Williamsburg.
  • Ability to pass a federal background check.
  • Willingness and financial ability to complete four unpaid phases of the pre-screening hiring process. Full compensation will begin upon successful completion. Details to be discussed during the screening interview.
  • Up-to-date PPD/Tuberculosis screening.
  • Completion of 10 Continuing Education Units (CEUs) annually.

Job Types: Full-time, Part-time

Pay: $ $35.00 per hour
