Role Description:

This is a full-time remote role for a Patient Care Coordinator. The coordinator will be responsible for managing patient appointments, ensuring timely communication, facilitating care coordination, and providing administrative support. Key day-to-day tasks include appointment scheduling, educating patients on care plans, conducting follow-ups, addressing inquiries, coordinating comprehensive care efforts, and maintaining accurate patient-related documentation.

Qualifications:

• Proficiency in Appointment Scheduling and strong Phone

Etiquette skills for effective communication

• Experience in Patient Care and Care Coordination to ensure seamless and holistic patient management

• Understanding of Medical Terminology to facilitate communication with healthcare providers and patients

• Strong organizational and time-management skills to handle multiple tasks

• Excellent interpersonal and communication abilities, both written and verbal

• Ability to work independently in a remote environment

• Previous experience in healthcare or patient care roles is a plus

• High school diploma or equivalent required; additional certifications in healthcare administration or related fields are advantageous
