Position Summary

The Activity Coordinator develops, leads, and documents structured daily activity programming that supports the physical, cognitive, emotional, cultural, and social well-being of participants (18+), with a primary focus on older adults. This role ensures engaging, person-centered recreational and therapeutic activity services in compliance with California ADHC/CBAS regulations, Health and Safety standards, and Home and Community-Based (HCB) Settings requirements. The Activity Coordinator works collaboratively with the interdisciplinary team (Administrator, RN, Social Work, OT/PT/ST, and program staff) and maintains same-day documentation for services provided.

A. Qualifications

The Activity Coordinator must meet all applicable California Title 22 and CBAS certification standards, including:

  • 18 years of age or older.
  • High school diploma or equivalent required; Associate's or Bachelor's degree in Recreation, Gerontology, Psychology, Social Services, or related field preferred.
  • Minimum of one (1) year experience in activity programming for seniors or adult day health care populations.
  • Ability to plan and lead group and individualized activities that support cognitive stimulation, physical movement, socialization, and emotional wellness.
  • Current CPR and First Aid Certification.
  • TB clearance within the past 12 months or obtained within 7 days of hire.
  • Pre-employment health screening signed by a physician confirming ability to perform job duties.
  • DOJ/FBI Live Scan background clearance required prior to employment.
  • Ability to stand, set up activity spaces, lift up to 25 lbs, and assist participants with mobility or fall-risk awareness when needed.
  • Ability to demonstrate competence through work history and verifiable references.
  • Must be reliable, organized, creative, patient-centered, and able to follow written care plans and center policies.
  • Ability to communicate effectively in English; bilingual (Spanish or Armenian) a plus.
  • Willingness to comply with participant rights, HIPAA, infection control, emergency preparedness, and HCB person-centered care standards.

B. Duties and ResponsibilitiesActivity Program Development & Coordination

  • Design, implement, and lead a monthly and weekly activity calendar that supports participant interests, abilities, and cultural sensitivity.
  • Provide individualized engagement opportunities based on participant preferences and care plan goals.
  • Lead group activities including, but not limited to:
  • Bingo, arts & crafts, music therapy, cognitive games, chair exercises, relaxation, social groups, seasonal events, and therapeutic recreation.
  • Ensure activities support HCB settings choice and autonomy, allowing participants to engage in activities of their choosing and interact with peers freely.
  • Coordinate activity support with center nursing and therapy staff when programs include physical or cognitive maintenance components.

Documentation & Compliance

  • Document all activity services on the same day they are provided, including participant participation notes when assigned.
  • Maintain required activity service logs, attendance, and outcomes tracking per center policy.
  • Ensure activity areas are physically accessible, safe, clean, and organized before and after each session.
  • Maintain supply inventory and submit timely requests for activity materials and program needs.

Participant Safety & Support

  • Supervise participants during program hours, ensuring fall-risk awareness, dignity, inclusion, and safe engagement.
  • Report any participant changes, concerns, or incidents to the RN or Administrator immediately.
  • Provide encouragement and redirection with a calm and supportive approach when needed.

Interdisciplinary Team Collaboration

  • Participate in care plan and program planning discussions as requested by the Administrator or RN.
  • Work closely with Program Aides and Drivers (where applicable) to ensure participant transition into activities upon arrival and support during center events.
  • Train and guide program staff on safe activity facilitation and participant engagement expectations.

Other Duties

  • Perform additional duties as assigned by the Administrator or supervisors.
  • Attend mandatory in-service training to remain knowledgeable in ADHC/CBAS laws, participant rights, HCB standards, emergency procedures, and center policies.
