Job Title: Shelter Case Manager (Nassau County)

Location: Hybrid / Remote (Based in Nassau County, NY)

Since 2019, our team has been dedicated to restoring lives through our work at Safe Haven Sober Living. We have a 6-year history of providing a foundation of housing, food, and employment for individuals in recovery, maintaining high standards of accountability through structured support and weekly toxicology screenings.

As we expand our mission to open a new emergency shelter in Nassau County, we are looking for a disciplined, mission-driven Case Manager to help our residents transition from crisis to permanent housing.

The Role

We are seeking a Case Manager who is a self-starter and capable of managing a caseload with a high degree of independence. While this role offers the option to work from home for administrative and planning tasks, it requires a "boots on the ground" mentality for resident success.

Key Responsibilities:

• Case Oversight: Develop and implement Individualized Service Plans (ISPs) to move residents toward permanent housing.
• Accountability: Oversee resident progress, ensuring adherence to house rules and program goals.
• Compliance: Maintain digital case files in strict accordance with Nassau County DSS and New York State OTDA (Part 491/900) standards.
• Reporting: Send detailed weekly updates to the Program Manager regarding resident milestones, incidents, and housing placements.
• Advocacy: Coordinate with local agencies for medical, mental health, and vocational services.

Requirements

• Regulatory Knowledge: Prior familiarity with Nassau County DSS regulations and OTDA standards is highly preferred. (Candidates without prior experience must be prepared to undergo intensive review and training on these standards).
• Experience: Proven track record in social services, case management, or supportive housing (Experience in sober living or recovery environments is a plus).
• Communication: Exceptional written communication skills for documentation and remote manager updates.
• Accountability: Ability to manage your own schedule and meet deadlines without direct daily supervision.
• Tech Savvy: Proficiency in digital case management software and remote communication tools (Zoom, Slack, or similar).

Pay: $ $25.00 per hour

Expected hours: 20 – 40 per week
