About LEAD

Learning Empowerment to Able the Disabled (LEAD) is a grassroots nonprofit based in Malheur County, Oregon. We are committed to empowering individuals with disabilities and advancing access, equity, and inclusion across rural communities. With new funding from the Oregon Health Authority, LEAD will expand our work connecting underserved residents to Oregon Health Plan (OHP) coverage through culturally responsive outreach, enrollment support, and healthcare system navigation.

Position Summary

LEAD is seeking a part-time Outreach Coordinator to lead outreach and enrollment activities under our OHP Outreach and System Navigation Program. This position will work across four Eastern Oregon counties to reach uninsured and underinsured individuals—particularly those in rural, immigrant, Indigenous, and disability communities—and help them access and navigate their OHP benefits.

This is a contract-based role averaging 15 hours per week. The position involves regional travel and requires frequent community engagement, as well as strong cultural awareness and the ability to build trust with historically underserved populations. Bilingual Spanish/English candidates are strongly encouraged to apply.

Key Responsibilities

Outreach & Community Engagement

  • Plan and conduct outreach in public spaces (e.g., libraries, schools, senior centers, grocery stores).
  • Build trusted relationships with community partners, including tribal health services, schools, clinics, and advocacy groups.
  • Distribute OHP materials in accessible, bilingual formats, ensuring ADA compliance.

Enrollment Assistance

  • Provide one-on-one application support using the Oregon Eligibility (ONE) system.
  • Assist individuals in understanding eligibility, gathering documents, and completing applications.
  • Conduct proactive follow-up with individuals who begin but do not complete enrollment.

System Navigation Support

  • Help enrollees find OHP providers, schedule appointments, and use benefits effectively.
  • Refer clients to social services, telehealth options, and community supports as needed.
  • Maintain strong relationships with FQHCs, Lifeways, and other local providers.

Reporting & Compliance

  • Maintain accurate, confidential service records and submit required monthly reports.
  • Complete and maintain OHA-required certification as an Application Assister or Community Health Worker (training provided).
  • Comply with all LEAD and OHA policies on privacy, accessibility, and ethics.

Compensation & Travel Reimbursement

  • Compensation will be negotiated based on experience.
  • Travel is reimbursed at GSA mileage rates.
  • Per diem lodging and meals are covered for approved travel.
  • Up to 12 overnight trips are expected during the anticipated 14-month contract period.

Qualifications

Required

  • Strong interpersonal and cross-cultural communication skills.
  • Familiarity with rural health and social service access barriers.
  • Reliable personal vehicle and valid auto insurance.
  • Ability and willingness to travel regionally, including occasional overnight stays.

Preferred

  • Minimum 2 years of experience in outreach, case management, public health, or social services.
  • Bilingual in Spanish and English.
  • Experience working with individuals with disabilities, tribal communities, immigrants, or low-income populations.
  • Knowledge of OHP, Medicaid, or other public benefit programs.
  • Current OHA Application Assister or CHW certification (or willingness to complete upon hire).
