At Hunter Creek Health and Rehabilitation in Northport, AL, we're seeking enthusiastic LPN Charge Nurses. You’ll be responsible for delivering direct resident care, mentoring CNAs, administering treatments and medications, and maintaining accurate clinical documentation. This role offers both professional growth and the opportunity to make a real difference in a caring, team‑oriented environment.

Shift:

1. Evening Shift, 3pm-11pm, Full-Time including rotating weekends
2. Night Shift, 11pm - 7am, Full-Time rotating weekends

Your Role as an LPN Charge Nurse:  

1. Deliver exceptional nursing care to residents according to care plans 
2. Administer medications and treatments with accuracy and efficiency 
3. Monitor and report changes in resident conditions to the clinical team 
4. Supervise and support CNAs and care staff during the shift 
5. Maintain resident documentation and uphold clinical policies 

Qualifications:  

1. Current and active LPN License  
2. Strong leadership, organizational, and communication skills 
3. Commitment to compassionate, person-centered care
