&##Licensed Practical Nurse (LPN)
As an LPN , you will provide direct nursing care to residents while collaborating with RNs, physicians, and the interdisciplinary care team. You’ll play a key role in supporting rehabilitation goals, ensuring comfort, safety, and dignity in a long-term care setting.
✅ Monitor, document, and report changes in residents’ health status, including vital signs
# Assist residents with activities of daily living (ADLs) as needed
# Collaborate with rehabilitation team members to support recovery goals
# Maintain accurate and timely documentation in compliance with facility policies and regulations
# Provide emotional support and education to residents and families
# Ensure a clean, safe environment by following infection control protocols
&##Current and valid Licensed Practical Nurse (LPN) license
# Strong clinical skills, including medication administration and wound care
