*Sage Creek Post Acute *Is looking for a compassionate, committed, and skilled *Speech Therapist *to join our team!
*Purpose: *To provide the highest quality of speech therapy services to our residents/patients consistent with established standards of practice and facility procedures.
*Duties and Responsibilities:*
* Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
* Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
* Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
* Establish individual resident therapy schedules in cooperation with other team members
* Conduct therapy according to established treatment plan and ensure proper documentation and billing of therapy services
*Requirements and Qualifications:*
* Active state license as a speech-language pathologist
* Current BLS/CPR certification
* At least 1 year of proven experience as an SLP within the last 5 years
*Physical Capacities*: (With or Without the Aid of Mechanical Devices)
* Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met.
* Must be able to move intermittently throughout the workday.
* Continuous bending, squatting, twisting and reaching.
* Must meet the general health requirements set forth by the policies of this facility which includes an annual TB screening and physical examination.
* Must be able to stand for and walk for extended periods of time.
* Must be able to lift and carry up to 25 pounds.
* Requires infrequent lifting of weight up to 100+ pounds with assistance.
* Frequent pushing and pulling of carts and transporting residents.
* Must be able to cope with the mental and emotional stress of the position.
*Environmental conditions: *Inside work, normal temperatures, some noise, occasional fumes/odors, and potential exposure to bloodborne pathogens.

Pay: $46.00 - $53.00 per hour
