Licensed Practical Nurse (LPN) – Community Skilled Health Care Centre

Community Skilled Health Care Centre is now hiring Licensed Practical Nurses (LPNs) to join our dedicated nursing team. As an LPN in our skilled nursing facility, you will play a key role in delivering high-quality resident care and supporting a strong healthcare team in a welcoming, well-managed environment.

• Provide direct nursing care to residents in a long-term care and skilled nursing setting
• Administer medications and treatments as prescribed by physicians
• Monitor residents' conditions, recognize changes, and report to the nursing supervisor or RN
• Collaborate with RNs, STNAs, and other healthcare professionals to ensure quality, patient-centered care
• Maintain accurate, detailed medical records and documentation in compliance with regulations
• Support rehabilitation and restorative nursing care programs
• Participate in care planning and assist with care coordination

• Active Licensed Practical Nurse (LPN) license in Ohio
• Strong clinical, communication, and critical thinking skills
• Ability to work effectively in a team nursing environment
• Previous long-term care or rehabilitation nursing experience preferred, but not required
