Social Services Director

At Brownsburg Health Care Center, we believe exceptional care starts with compassionate people. We are seeking a Social Services Director to support the emotional, social, and psychological well-being of our residents through leadership, advocacy, and care planning.

• Complete psychosocial assessments and create individualized care plans
• Provide emotional and mental health support to residents and families
• Lead the discharge planning process and coordinate community referrals
• Uphold and advocate for residents' rights and dignity
• Collaborate with nursing and interdisciplinary care teams
• Maintain timely, accurate documentation that meets all state and federal requirements
• Participate in regulatory survey preparation and execution
• Support or supervise social services staff, if applicable

• Required: Bachelor's Degree in Social Work or related Human Services field (Psychology, Sociology, Rehab Counseling, etc.)
• Required: Certified, licensed, or registered to practice in Indiana
• Required: 1 year of supervised social work in a healthcare setting (geriatrics preferred)
• Preferred: 2+ years of experience in a Skilled Nursing Facility
