We're seeking an MDS Coordinator to join our skilled nursing facility team. In this role, you'll be responsible for assessing resident health, completing accurate documentation, and developing individualized care plans that promote quality outcomes and regulatory compliance. Qualifications

• An active LPN or RN license in the state of Wisconsin.

Key Responsibilities

• Coordinate and complete MDS assessments accurately and on schedule.
• Manage new admission authorizations and ongoing communication with families, case managers, and insurance providers.
• Maintain and update ICD-10 coding for new admissions and long-term care residents.
• Develop, implement, and review individualized resident care plans.
• Lead Utilization Review meetings and validate PDPM scores and reimbursement accuracy.
• Educate and support the interdisciplinary team (IDT) on documentation and skilled service requirements.
• Monitor Quality Measures and participate in QAPI initiatives.
• Conduct chart audits and assist the IDT during survey processes.
