Youth Shelters & Family Services is seeking a compassionate and dedicated Case Manager to join our Transitional Living Program team, empowering individuals on their journey to self-sufficiency and independence.

What would my job responsibilities be?

  • Provide comprehensive case management services from screening and intake, including client assessments, development of self-sufficiency plans, coordination of services, and discharge and after-care planning.
  • Collaborate with clients to identify personal goals related to education, employment, housing, mental health, and substance abuse recovery, and develop action plans to achieve these goals.
  • Deliver individualized life skills instruction to TLP clients in accordance with established self-sufficiency plans and conduct daily life skills groups covering various topics relevant to independent living.
  • Organize and facilitate workshops and group sessions on topics relevant to independent living, such as financial literacy, nutrition, health and wellness, job readiness, and interpersonal skills.
  • Advocate on behalf of clients to access needed services and resources, such as housing assistance, educational opportunities, legal aid, and transportation services.
  • Maintain collaborative relationships with external stakeholders, including social service agencies, educational institutions, employers, and community organizations, to enhance service coordination and access to resources for clients.
  • Participate in program evaluation efforts by collecting client feedback, identifying areas for improvement, and contributing ideas for enhancing program effectiveness and client outcomes.
  • Engage in daily activities of the program, including oversight of facility and grounds maintenance, vehicle and equipment upkeep, and record-keeping related to program operations.
  • Participate in the emergency on-call system as assigned, providing timely response and support to program participants during crisis situations to ensure their safety and well-being.
  • Maintain accurate and up-to-date records of participant progress, service provision, and outcomes in accordance with program and funding requirements.
  • Gather and report program data, including HMIS reporting and funders' mandated data, assisting in the completion of monthly and annual report forms to fulfill reporting obligations and program evaluation needs.
  • Provide necessary information and data for required reports and contracts, contributing to program evaluation and accountability efforts.

What are the qualifications and experience I need to become a Case Manager?

  • Demonstrated experience in providing life skills training, vocational guidance, or related services to vulnerable populations, with a particular focus on homeless youth or young adults.
  • Proficiency in trauma-informed care principles, motivational interviewing techniques, and evidence-based practices for supporting youth in transition.
  • Excellent communication and collaboration skills, with the ability to effectively engage with diverse individuals and groups, including youth from different cultural backgrounds, LGBTQ+ individuals, and individuals with disabilities.
  • Preferred experience in program development, curriculum design, or workshop facilitation, enhancing the ability to create engaging and impactful programming for program participants.
  • Strong knowledge of community resources and social service systems relevant to housing, education, employment, healthcare, mental health, and substance abuse treatment, facilitating effective referrals and support for program participants.
  • Exceptional interpersonal skills, including empathy, active listening, conflict resolution, and the ability to set and maintain boundaries to ensure the well-being of program participants.
  • Flexibility to adapt to changing priorities, work independently with minimal supervision, and collaborate effectively within a multidisciplinary team environment, contributing to the overall success and effectiveness of the program.
  • Bilingual in Spanish and English.

Required

  • CPR/ First Aid Certified (will train)
  • Must be able to drive company vehicles.
  • Valid Driver's License
  • Must be over 21 years of age to meet the minimum eligibility requirements of our automobile insurance carrier.

What are the minimum education requirements?

  • Bachelor's Degree in human services related field preferred and at least two years related experience
  • OR High School diploma/GED with two years relevant experience.
