*Job Summary*
We are seeking a dynamic and compassionate Licensed Practical Nurse (LPN) to step into the vital role of Charge Nurse within our healthcare team. In this energetic leadership position, you will oversee daily patient care operations, coordinate nursing staff, and ensure the delivery of exceptional medical services. *Responsibilities*
* Supervise and coordinate nursing staff to ensure high standards of patient care are maintained across all shifts.
* Conduct comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations, to inform treatment plans.
* Administer medications, injections, IV infusions, and perform catheterizations following strict protocols and HIPAA guidelines.
* Oversee medical documentation using EMR (Electronic Medical Records)
* Manage complex cases involving acute pain management, dementia care (including Alzheimer’s and memory care), and infectious disease protocols.
* Collaborate with multidisciplinary teams long-term care facilities
* Ensure infection control practices are rigorously followed and maintain sterile processing standards for all clinical procedures.
* Support discharge planning, case management activities, and patient education to promote optimal recovery outcomes.
*Experience*
* Valid Licensed Practical Nurse (LPN) license with current certification in basic life support (BLS).
* Prior experience with EMR/EHR systems highly desirable for efficient documentation workflows
* Knowledge of physiology, anatomy, pharmacology related to pain management, tube feeding, catheter care, and medical terminology is essential.
* Experience administering injections, performing venipuncture or phlebotomy procedures can be useful.
* Background working with populations such as seniors (geriatrics), individuals with dementia (including Alzheimer’s and memory care).
* Familiarity with infection control protocols, sterile techniques (aseptic technique), and post-acute or home & community care experience will set you apart as a candidate ready to excel in this role.
