Job Summary

Care Center Home Health is seeking a compassionate Home Health LPN to provide skilled nursing care under the supervision of a Registered Nurse. This role includes assisting with patient care plans, performing approved nursing procedures, documenting care, and ensuring patient safety and comfort in the home setting.

Compensation: Starting rate of $36 per visit

Key Responsibilities

• Provide skilled nursing care as directed by the RN and physician's orders.
• Assist with implementing, evaluating, and updating patient care plans.
• Perform approved treatments/procedures (per policy and state regulations), including:
• Medication administration
• Wound care
• Catheter care
• Drain maintenance (under RN supervision)
• Monitor and report changes in patient condition, behavior, or symptoms.
• Prepare accurate clinical and progress notes (excluding admission notes).
• Educate patients and caregivers on self-care and safety techniques.
• Promote patient comfort, independence, and emotional support.
• Participate in team meetings, case conferences, and in-service trainings.
• Travel to patient homes and provide care in various home environments.

Job Conditions

• Must be able to communicate clearly in English (verbal and written).
• Must be able to safely access homes that may not be accessible to wheelchairs.
• Requires adequate hearing, vision, and physical dexterity for patient assessments.
• Physical activities include walking, standing, stooping, sitting, and assisting with lifting or turning patients.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Active LPN license in the state.
• 1+ year of home health or hospital experience preferred.
• Current CPR certification.
• Clean criminal background check.
• Not excluded from Medicare/Medicaid programs.
• Reliable transportation for home visits.
