We are currently adding Case Managers to our growing team

About the Socially Necessary Services Case Manager Role:

• This role will require strong documentation, communication, and resourcing skills.
• Create service and discharge plans.
• Complete individual and family assessments
• Create service plans to address client needs.
• Adhere to all current federal and state laws and regulatory requirements, as well as The Balm Center's policies and procedures.
• Work to maintain professional competence and skills via training and continuing education.
• Participate in case consultation and agency meetings.
• Ability to run psychoeducation and skills-building sessions in one-on-one or group settings with training.
• Work with DHHR CPS workers, Adoption workers, and Supervisors to provide case updates and attend MDT meetings.
• Participate in SNS services, including but not limited to transportation, supervised visitation, parenting training, home studies, and other duties as assigned.
• Maintain a flexible schedule to accommodate SNS service needs, including some weekend hours.
• Complete SNS Monthly reports
• Keep track of all SNS services provided and document services in progress notes.

Requirements for Socially Necessary Services Case Manager:

• High School Diploma or GED is required.
• A bachelor's degree in social work or a Human Services-related field is preferred.
• Eligible for Licensed Social Work (LSW) credential, preferred.
• Excellent written and verbal communication skills
• Experience with case management documentation and HIPAA Policy adherence.
• Ability to work in a clinical team and community settings, including homes.
• Being compassionate and understanding with the ability to adapt to the specific needs of each client.
• Familiarity with DHHR SNS services and reports
