We are currently adding Case Managers to our growing team
About the Socially Necessary Services Case Manager Role:
- This role will require strong documentation, communication, and resourcing skills.
- Create service and discharge plans.
- Complete individual and family assessments
- Create service plans to address client needs.
- Adhere to all current federal and state laws and regulatory requirements, as well as The Balm Center's policies and procedures.
- Work to maintain professional competence and skills via training and continuing education.
- Participate in case consultation and agency meetings.
- Ability to run psychoeducation and skills-building sessions in one-on-one or group settings with training.
- Work with DHHR CPS workers, Adoption workers, and Supervisors to provide case updates and attend MDT meetings.
- Participate in SNS services, including but not limited to transportation, supervised visitation, parenting training, home studies, and other duties as assigned.
- Maintain a flexible schedule to accommodate SNS service needs, including some weekend hours.
- Complete SNS Monthly reports
- Keep track of all SNS services provided and document services in progress notes.
Requirements for Socially Necessary Services Case Manager:
- High School Diploma or GED is required.
- A bachelor's degree in social work or a Human Services-related field is preferred.
- Eligible for Licensed Social Work (LSW) credential, preferred.
- Excellent written and verbal communication skills
- Experience with case management documentation and HIPAA Policy adherence.
- Ability to work in a clinical team and community settings, including homes.
- Being compassionate and understanding with the ability to adapt to the specific needs of each client.
- Familiarity with DHHR SNS services and reports
