Summary
The Behavioral Health Social Worker MSW plays a pivotal role in carrying out the mission of the organization providing high quality care including treatment planning, discharge planning, and utilization review to patients hospitalized on the mental health unit. This role is responsible for assessing psycho?social needs, educating, counseling, along with planning, coordinating, and implementing care across the continuum while contributing to patient outcomes and length of stay. Behavioral Health MSW works closely with other professionals (i.e., nursing, counselling staff, occupational therapy, psychiatry, psychology) Communicates teamwork, caring, and compassion with all internal and external healthcare team members.
Responsibilities

Complete and document a standard social work psycho social assessment by:
Eliminate Barriers to treatment increase patient/family satisfaction, improve appropriate utilization of resources, create safety plan to address ethical and legal concerns, discharge and transitional care needs, trauma, financial problems, treatment compliance issues, substance use and other psychosocial barriers to maximizing health status.

Provide social work intervention as it relates to hospitalization including crisis management, mental health and suicide resources, health care decision?making, illness, ethical/legal concerns, discharge planning, and transitional care needs abuse, domestic violence, capacity, financial problems, compliance issues, as well as other psychosocial barriers to maximizing health status.

Actively participate in care coordination efforts to successfully identify high?risk factors and respond appropriately so that key information along with the ability to co?lead daily care coordination meetings with care team.

Work with the healthcare team, patient, family and/or significant others to continually transition the patient to the appropriate level/place of care and avoidable days are captured, documented, advocate, mediate, and negotiate, with an emphasis on self determination for the patient system. Formulate a cohesive plan for maintaining the patients health status, improving social supports, and moving the patient safely into less restrictive, less costly levels of care according to available resources. Meet the comprehensive needs of the patient in their environment to avoid readmission.

Integrate relevant theories of family dynamics, crisis intervention, strengths based, solution focused and meaning of illness into every work with patients.
Participate in care coordination efforts to identify high?risk factors and respond appropriately so that key information is captured and documented. Participates in daily team meetings.
Collaborates care with outside agencies, set F/U appointments, work with CMH for additional resource assistance with placement and discharge plan.
Builds professional relationships with patient, families and the treatment team to address conflict resolution with positive outcomes.
Communicates teamwork, caring and compassion
Provides excellent customer service in accordance with the mission, vision and values of the health system.
Assume responsibility for continuing professional development and maintains licensure.
Provides for the age specific needs of the population served according to evidence based standards and organizational policies/procedures for the adult and older adult populations.
Other duties as assigned.
Membership in at least one professional organization (i.e. NASW, SSWLHC?MI Chapter, national SSWLHC, etc.) preferred.
