Job Title: Physical Therapist (PT) - Long Term Care (LTC)

Job Summary:

We are seeking a dedicated Physical Therapist to join our team and provide specialized care to patients in Long Term Care (LTC) facilities. The ideal candidate will have a passion for helping individuals improve their mobility and quality of life.

Responsibilities and Duties:

• Evaluate patients' physical condition and create individualized treatment plans
• Implement therapeutic exercises and modalities to improve strength, flexibility, and endurance
• Educate patients and their families on proper techniques for safe and effective movement
• Monitor progress and adjust treatment plans as needed
• Collaborate with other healthcare professionals to ensure comprehensive care for patients

Qualifications and Skills:

• Graduate of an accredited physical therapy program
• Valid state PT license
• Experience working in Long Term Care (LTC) settings preferred
• Strong communication skills and a compassionate demeanor
• Ability to work independently and as part of a team
