Job Overview
We are seeking a dedicated and compassionate Care Coordinator to join our healthcare team. In this vital role, you will serve as a key point of contact for patients, providers, and support staff, ensuring seamless coordination of care services. Responsibilities

  • Act as the primary liaison between patients, healthcare providers, and support teams to coordinate appointments, follow-ups, and care plans
  • Assist patients with scheduling, rescheduling, and confirming appointments across various medical departments
  • Maintain accurate and organized patient records, ensuring confidentiality and compliance with privacy regulations
  • Provide administrative support by managing phone calls, emails, and documentation related to patient care
  • Collaborate with medical staff to facilitate communication and ensure timely delivery of services
  • Educate patients on treatment procedures, medication instructions, and available resources to promote understanding and adherence
  • Monitor patient progress and follow up on outstanding needs or concerns to enhance overall care quality

Requirements

  • Proven experience in a medical office environment or healthcare setting
  • Strong knowledge of medical terminology and healthcare procedures
  • Excellent organizational skills with the ability to manage multiple tasks efficiently
  • Proficiency in medical administrative support tools such as electronic health records (EHR) systems and office software
  • Prior experience working in a dental or general medical office is highly desirable
  • Ability to communicate clearly and empathetically with diverse patient populations
  • Knowledge of medical office protocols and compliance standards
