Overview
We are seeking a dedicated and compassionate Physical Therapist to join our team. The ideal candidate will be responsible for evaluating, planning, and implementing individualized treatment programs for patients with various physical conditions. This role requires a strong understanding of anatomy, patient care, and therapeutic techniques to enhance patients' mobility and quality of life. A physical therapist working in a home health setting will collaborate with other healthcare professionals to provide patient-centered care, focusing on improving functional goals and increasing independence with the patient's home environment.

Duties

• Conduct comprehensive evaluations of patients to assess their physical abilities and limitations.
• Develop and implement personalized care plans that address the specific needs of each patient.
• Utilize various therapeutic techniques including trigger point therapy, orthopedic rehabilitation, and discharge planning to facilitate recovery.
• Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.
• Educate patients and their families on exercises, home care strategies, and lifestyle modifications to support recovery.
• Collaborate with other healthcare professionals to provide integrated care for patients.
• Maintain accurate documentation of patient treatments, progress notes, and outcomes in compliance with clinic policies.

Qualifications

• Possesses a degree from a bachelorette or masters program in physical therapy approved by an accredited organization.
• Licensed to practice as physical therapist within the state, if applicable.
• Two years of appropriate experience as a physical therapist. Community/home health experience is preferred.
• Demonstrates good verbal and written communication and organization skills.
• Possesses and maintains current CPR certification.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Commitment to providing high-quality patient care in home health settings.

Area of coverage: San Joaquin County, CA

Salary: Pay is per visit NOT per hour

Status: Per diem or On call- choose your own times and days.

Pay: From $180.00 per hour
