The Oxford County Community Health Centre's (OCCHC) serves complex, marginalized populations who face significant barriers in accessing primary care with a full range of wraparound team-based services. For our Woodstock site, we are seeking a Full Time:
NURSE PRACTITIONER – Primary Care
Overview of Position: As a member of our broad interdisciplinary team, the NP will provide comprehensive primary care services to registered clients with a focus on those who are high risk and/or experiencing barriers to accessing services. The NP Primary Care is responsible for health assessments, diagnosis, therapeutics, health education /counselling, disease prevention, & other medical functions. The OCCHC supports all disciplines working within a full scope of practice.
Compensation & Benefits: Up to $135,000.00 with comprehensive benefits including HOOPP.
Hours of Work: 8:30am – 4:30 pm, Monday to Friday, no weekends.
Requirements:
- Master's Degree in Nursing with a Primary Care NP designation or Baccalaureate Nursing degree with an extended class diploma, from a recognized university.
- Current Registration as an RN-EC and a member in good standing with the CNO.
Preference will be given to candidates with:
- Demonstrated experience and comfort-level working with vulnerable populations including individuals with mental health challenges, substance use disorder, experiencing homelessness, etc.
- Demonstrated ability to work independently and collaboratively within an interprofessional team.
- Community experience, preferably in primary health care or combination of community, hospital and/or public health.
- Strong communication & interpersonal skills, combined with excellent critical thinking, problem solving and organizational skills.
- Strong aptitude for "meeting clients where they are at" – assisting clients in directing their own journey.
- Strong computer skills including previous experience with an Electronic Health Record (PS Suite an asset).
