Schedule: Full-Time, Monday Through Friday, 8:00 – 4:30 onsite. May require occasional flexed schedule to accommodate special events.

What is PACE?
PACE, or Program of All-Inclusive Care for the Elderly, is an alternative care model for those over 55 years of age requiring nursing home level of care which aims to keep participants living at home in the community. PACE is a collaborative, interdisciplinary approach which coordinates all levels of medical and social needs, including prevention, wellness, socialization, primary care, and access to specialists and other services.
The Panorama PACE Care Scheduler serves as the liaison between participants, the PACE Interdisciplinary Team (IDT), and provider partners for scheduling of outside appointments. This role requires strong customer service and organizational skills. The position requires the ability to multitask and execute complex scheduling and administrative tasks to support the operations of Panorama PACE (Program of All-Inclusive Care for the Elderly).
Primary Duties and Responsibilities:
- Serve as the primary liaison with external vendors and provider offices to manage all outside clinical appointments for PACE participants, ensuring timely and efficient access to care.
- Coordinate with families/caregivers and the Transportation Coordinator to ensure transportation is aligned with scheduled appointments.
- Communicate appointment details clearly to participants and caregivers, addressing questions related to scheduling, transportation, and follow-up care.
- Document and track appointments accurately in external systems and the PACE EHR, including, but not limited to: managing referral workflow (e.g., submission of referrals and authorizations), appointment outcomes, follow-up needs, and related communications, and ensure relevant information is communicated to IDT members.
- Maintain organized records related to appointments, vendor communications, and participant information, ensuring timely updates and compliance with regulatory requirements.
- Maintain knowledge of Panorama PACE program services to provide accurate information.
- Attend and participate in employee meetings.
- Support quality and compliance efforts by ensuring proper documentation and regulatory adherence.
The Care Scheduler will provide cross coverage and support for Office Coordinator functions as required:
- Manage front desk activities, including greeting participants, visitors, and staff with professionalism and warmth.
- Answer and direct incoming calls, providing accurate information or routing inquiries to appropriate departments.
- Assist with data entry, filing, and document preparation to support smooth daily operations.
Requirements:
- Bilingual English/Spanish fluency is required.
- High school diploma or equivalent; associate's degree preferred.
- Previous experience in a healthcare setting or with geriatric populations is highly desirable.
- Familiarity with scheduling software and EHR systems (e.g., EPIC, CarePlanner) preferred.
- One year of experience at a reception desk, particularly in a clinical environment, is highly preferred.
- One year of experience with patient scheduling is highly preferred
- Basic level of medical terminology preferred
- Strong interpersonal and communication skills to interact effectively with participants, families, staff, and outside vendors.
- Excellent organizational and multitasking abilities to manage front desk operations and scheduling simultaneously.
- Attention to detail to ensure accurate scheduling and record-keeping.
- Proficiency in Microsoft Office Suite and basic office equipment.
- Ability to handle sensitive information with confidentiality and professionalism.
