- Maintains stocked supplies in patient care area
Job Responsibilities:
- Prepares patients for visit by completing a through clinical intake including vital signs, history, and assessment
- Assign patients to appropriate treatment protocols/locations and informing physician of patient status and location in order to control patient flow
- Administers medication as directed by provider and handles prescription refills as delegated by provider
- Performs venipuncture/ capillary punctures per protocol when ordered
- Arranges for patient testing and admissions as directed by ordering provider
- Provides patient education related to scheduled procedures, test results, medications, or other self-care management to make sure patient understands the care being provided
- Responds to inbound and outbound phone calls and portal messages according to policy
- Maintains patient confidentiality at all times according to HIPAA requirements
- Participates in professional development activities appropriate for active licensure and professional affiliation
- Stays current with established office practice protocols and Hamilton Health System Educational requirements
- Provide input and feedback on clinical protocols and equipment
- Maintains accurate charting and completes pre authorizations as required for care
- Completes orders as directed by providers
Leads and Maintains positive communication to other associates related to patient care and patient safety:
- Attends 75 % of staff meetings and presents any information related to clinical care
- Attends 75 % of the clinical leader meetings and shares information with other associates
- Leads daily huddles to communicate practice and patient needs daily
- Provides communication to other associates related to new equipment or policy changes related to clinical care
- Provides interdepartmental communication about patient care as needed
- Continually monitors patient and provider workflow and makes adjustments as needed to improve care and efficiency
- Clinical leaders serve as a resource and change agent for clinical associates
- Clinical leaders assist with conflict resolution and escalate issues as needed
Supervises the Clinical Staff and Maintains a safe clinical environment :
- Provides oversite, education and management of in house and sample medication in the practice to include proper documentation related to receiving and dispensing medications in order to maintain safety and meet regulatory requirements
- Maintains daily checks on patient equipment to make sure it is functioning properly and safe to use
- Maintains a departmental clinical competency check off list for associates and departmental specific policies / protocols
- Accountable for onboarding new clinical staff to make sure they meet the clinical competency requirements
- Clinical leaders are responsible for appropriate documentation to be completed within 90 days of new clinical staff
- Clinical leaders assist with coordinating and supporting nursing students to make sure they have a positive experience
- Clinical leaders support the practice manager and assist with issues or education needed for the clinical staff
- Clinical leaders evaluate, identify and implement action plans to improve workflow process and provide feedback to the Clinical Quality Manager about improvement opportunities in the practice and across all practices.
- Clinical leaders provide an outline of annual competencies to be completed by the practice staff annually
- Demonstrates problem solving ability by readily identifying and addressing any disruptive situation, process issue or operations concern
- Communicates well with all groups including associates, physicians and patients
- Identifies and addresses any patient issue or concern to ensure resolution
- Provides clinical associate feedback for evaluations
- Directly communicates with other clinical leaders about interdepartmental workflows that need improvement
JOB QUALIFICATIONS
Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
Licensure: Current LPN License in the State of Georgia, Current CPR Certification
Experience: 1 year in a physician office practice or hospital preferred.
Skills: Nursing skills as defined in the Law Governing the Practice of Nursing in Georgia. Knowledge of clinical patient care, the ability to use critical thinking skills, highly organized with the ability to maintain accurate documentation and reporting, demonstrates evidence of exceptional leadership skills, positive communication, and collaboration with patients, families and other health care team members, maintains core values that are consistent with quality patient/family centered care, and maintains a positive attitude toward other team members.
