Duties

• Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor and record vital signs such as blood pressure, pulse, and temperature to ensure health stability
• Provide medication reminders and assist with medication administration following HIPAA guidelines
• Support residents with dementia or Alzheimer's by implementing specialized memory care techniques and ensuring safety
• Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary
• Maintain cleanliness of living areas through regular cleaning and sanitation practices
• Use assistive devices such as Hoyer lifts and resident's to ensure safe transfers and mobility support
• Observe and report changes in residents' physical or emotional condition to healthcare professionals or family members
• Follow care plans meticulously to deliver consistent, high-quality patient care in home or community settings
• Engage in patient monitoring during home health visits or hospice care, ensuring comfort and safety at all times

Experience

• Prior experience working in assisted living, nursing homes, or group homes is highly preferred
• Demonstrated knowledge of patient observation, vital signs monitoring, and medication administration
• Experience providing Alzheimer's care, dementia care, or memory care is advantageous
• Familiarity with HIPAA regulations to ensure confidentiality and privacy of patient information
• Hands-on experience with patient
