Responsibilities:
* Evaluate patient's physical, cognitive, and psychosocial abilities.
* Develop and implement treatment plans to improve patient's quality of life.
* Provide instruction to patients and families on how to use assistive devices and adaptive techniques.
* Monitor patient progress and modify treatment plans as needed.
* Collaborate with other healthcare professionals to coordinate care for patients.
* Educate patients on self-care strategies and techniques.
* Provide support to family members of patients.
