The Medical Case Manager will work as part of a specialized team within the Primary Care setting of Sun River Health to provide medical case management services to HIV+ patients enrolled in Ryan White Park C. Services include but are not limited to: assessing patient needs and barriers to care; service plan development; care coordination; referrals and linkages; and education and support by sharing effective strategies to manage their health; The Medical Case Manager will utilize motivational interviewing and a harm reduction approach to provide trauma informed care based supportive interventions. Additionally, the Medical Case Manager will provide assistance to individual not enrolled in case management, but in need of situational support.

Pay: $23.00 $25.24

Sign On Bonus: $1500

Essential Duties:

  • Will provide telephonic care coordination services to Medicare eligible patients.
  • These services will include coordination of medication refills, referral and diagnostic imaging results.
  • This position will be responsible for documenting all services in eCW as well as communication with internal clinical or behavioral health team.
  • This position will assist in linking patients to services for daily activities, food insecurity and home care assistance.

Minimum Experience & Education Requirements:

  • High School Diploma/Equivalency Required (Must Show Proof)
  • Associates/Bachelor's Degree (Preferred)
  • Must have working knowledge and familiarity with Medicare policies
  • Working knowledge of linkages to care and working with underserved communities.
