Job Description:

We are currently seeking a licensed and experienced Physical Therapist (PT) to provide in-home rehabilitation services as an independent contractor. This is an excellent opportunity for a motivated PT looking for flexible scheduling, autonomy in practice, and the ability to deliver personalized, impactful care directly in patients' homes.

You will assess, develop, and implement individualized therapy plans that promote optimal physical function, safety, and quality of life.

Key Responsibilities:

• Conduct initial and ongoing patient assessments and develop customized treatment plans.
• Deliver therapeutic interventions focused on mobility, strength, balance, and pain management.
• Educate patients and caregivers on home exercise programs and fall prevention.
• Document all clinical services accurately and in a timely manner per agency and regulatory requirements.
• Collaborate with physicians, nurses, and other members of the interdisciplinary team to ensure coordinated care.
• Participate in staff development and in-services.
• Notify physician and agency of any reports of the patient's condition.
• Provide supervision to Physical Therapist Assistants (PTAs) as needed.

Special Requirements:

• Must be able to constantly stoop and bend
• Must be able to lift and carry equipment up to 10 pounds up and down stairs.
• Must have reliable transportation and travel to prospective clients' residences.

Credential & Experience Requirements:

• Active Physical Therapist license in the state of California.
• Graduate of an accredited Physical Therapy program (DPT or MPT).
• Minimum 1 year of clinical experience and home health experience strongly preferred.
• Current CPR certification.
• Valid driver's license
• Auto insurance
• Liability insurance
• Proficiency with electronic documentation (EMR) systems.
• Excellent communication, clinical judgment, and time management skills.
