Full-Time Position 40 hours per week. Non-exempt
Reports to: Residential Manager/Supervisor or Designee
Overview
We are seeking a compassionate and experienced Residential Coordinator to oversee and support residents in a home and community care setting. The ideal candidate will have a strong background in senior care, assisted living, or long-term care environments, with a focus on providing personalized support to individuals with disabilities, dementia, or other special needs. Duties
- Coordinate daily activities and ensure residents' needs are met with dignity and respect.
- Assist residents with Activities of Daily Living (ADLs), including personal care, mobility, and hygiene.
- Administer medications accurately following prescribed protocols and document.
- Monitor vital signs and health status of residents, reporting any concerns to healthcare professionals.
- Provide specialized care for residents with implementing behavior management strategies.
- Support residents during meal preparation, feeding, and nutrition management.
- Maintain a clean and safe environment through light cleaning and sanitation tasks.
- Collaborate with healthcare providers, families, and team members to develop individualized care plans.
- Document resident progress and incidents accurately in compliance with regulatory standards.
- Support residents' social engagement activities to promote mental health and community involvement.
- Drive residents when necessary for appointments or outings, adhering to safety protocols.
Program Description:
Residential Habilitation: These services are indirect and direct services provided to individuals who live in a licensed or unlicensed provider owned rented or leased. This service is built on the principle that every individual has the capacity to engage in lifelong learning and assisting individuals with Intellectual and Developmental Disabilities utilizing a person centered approach.
General Description of the Responsibilities for the Residential Coordinator
The Residential Coordinator is responsible for the overall oversight and staff supervision of their designated residential homes. The Residential Coordinator completes and manages necessary individual and home documentation. They are also responsible to ensure the health, safety and welfare of the individuals they serve. This includes understanding and implementing CCCC's policies and regulatory requirements as it pertains to the residential program and staff oversight.
Residential Coordinator Duties:
· Promote and educate person centered planning and positive behavior supports.
· Attend required trainings and meet 24 hours of training a year.
· Be able to assist the individual in navigating the community and its resources.
· Assist individuals with self-care as needed.
· Assist with budgeting, grocery shopping, planning activities, housekeeping working on all skills necessary to help the individual gain independence in that area.
· Knowledgeable and implementation of and regulations as best business practice.
· Recognize and reporting Incident Management per the I.M guidelines.
· Adhere to Person Centered Planning and Positive Approaches.
· Develop, implement monitor weekly staff schedules.
· Manage the overall operation of their assigned home(s).
· Manage weekly staff schedules and PTO in order to promote efficiency as it pertains to fiscal feasibility and staff efficacy
· Review timesheet and turn into Fiscal Director.
· Responsible for home orientation of newly hired residential staff.
· Complete expense vouchers and house accounts and send to Residential Manager for review and approval
· Daily checks of all client service documentation for accuracy and completion
· Scheduling, preparing, and attending assigned Individual's appointments
· Order, administer and monitor individual's medications
· Ensure all items that need turned into the office are completed and bring any items for the home back
· Participate in weekly staff meeting and all ISP meetings for designated individuals
· Provide On-call and direct care duties as needed. (On call for all homes on a rotation)
· Ensure all end of monthly paperwork is accurate, complete, signed and ready for PS for designated individuals
· File accurate and applicable paperwork in the home
· Ensure designated Individual's appointment forms are sent to the PS
· Work as a team with the other Residential Coordinators and Direct Care Staff to promote teamwork through Residential Services and other agency entities.
· All duties deemed necessary by Residential Manager, Program Specialists, Risk Management/Human Resources, CEO, or other agency designee

· Ability to work flexible hours to meet program needs
· Ability to be flexible in working locations. There will be times when an individual has decided to move to another address. This change may require staff to relocate to the new location to perform their job duties
Education, Credential, and Experience:
Must have a high school diploma or a GED Equivalent
Requirements:
· Must be 18 years or older
· Must have a valid driver's license and a reliable vehicle
· Must have a physical and TB testing completed prior to employment
· Satisfactory clearance of a PA State Clearance Background Check (SP4-164) within 1 year of or FBI criminal Background check if residing out of state or having lived in the Commonwealth of PA less than 2 years.
