Job Description:

JOB SUMMARY

  • Provides care management for consumers receiving OPTIONS services, as well as those consumers appearing on the Office of Aging waiting list.
  • Works with consumer and family to assess need, develop and implement a safe and comprehensive Individual Service Plan (ISP).

REPORTING RELATIONSHIPS

  • This employee reports to and has frequent contact with supervisor concerning situations, care management issues, and work assignments.
  • Supervisory meetings are held monthly to review workload, and to discuss specific assignments, issues and concerns, and employee matters.
  • Supervisor oversees work by reviewing all case records in database, Individual Service Plans (ISPs), and other paperwork, by making joint supervisory visits as needed, and through observation of overall employee performance.

ESSENTIAL JOB FUNCTIONS

  • Completes comprehensive Individualized Service Plans (based on medical model and assessed need), documents according to requirements, interprets program policies and monitors consumer situations and provision of services on regular basis via home visits and phone contacts with consumers and caregivers to ensure that services meet the needs of the consumer. Includes coordinating services of formal contracted providers, formal non-contracted providers, and informal providers such as family or friends to complete worksheets and establish back up plans for consumers to ensure that care plan is safe and meets consumer needs. Communicates ongoingly with all providers of services to discuss consumer emergencies and needs, and to inform of any changes in care plans. (40%)
  • Completes required paperwork and data entry, including extensive case documentation, according to PA Department of Aging and agency standards for Options consumers (35%)
  • Provides Back Up Protective Service Investigative services, evaluate situation and provide services to reduce risk. * (25%)
  • In the course of performing job duties, employee will be required on a routine basis to provide own transportation to consumers homes, meetings and other job-related appointments and access those locations without special accommodations.

OTHER SPECIFIC TASKS OR DUTIES

  • Maintains accurate knowledge of all aging services and thorough working knowledge of community services.
  • Attends all mandated training sponsored by Department of Aging relative to community services and programs (including enrichment sessions).
  • Performs other duties as assigned.

MINIMUM QUALIFICATIONS

  • Successful completion of six (6) months of experience with the County as a Caseworker Trainee; or
  • Successful completion of the County Social Caseworker Intern Program; or
  • One (1) year of experience in public or private social work and a bachelor's degree which includes or is supplemented by 12 semester hrs credit in sociology, social welfare, psychology, gerontology or other related social sciences: or
  • A bachelor's degree with a social welfare major; or
  • Any equivalent combination of experiences and training including successful completion of 12 semester hrs credit in sociology, social welfare, psychology, gerontology or other related social sciences.

KNOWLEDGE, SKILLS AND ABILITIES

  • Current knowledge of HIPAA regulations and their interpretation is maintained and adhered to.
  • Give and receive information and feedback in order to provide a more complete result.
  • Respect for and awareness of others, good judgment in responding to conflict, and flexibility to adapt to changes in one's work environment.
  • Understand and anticipate customer needs.

REQUIRED LICENSES/CERTIFICATIONS/CLEARANCES

  • Acceptable Pre-Employment Criminal Background Check
  • Acceptable Pre-Employment Drug Screening
  • Acceptable Driver History in accordance with County Policy
  • Must successfully complete the Pennsylvania Department of Aging Protective Services Basic training by the end of the first year of employment and annual refresher trainings, thereafter.

PHYSICAL REQUIREMENTS/WORK ENVIRONMENT

  • This job is not sedentary, carrying or lifting rolling laptop case weight, approximate weight is 6 lbs.
  • Employee may be required to access multiple levels in dwellings without elevator access in order to access consumers.
  • Repetitive motion of data entry in the field and office onto laptop.
  • This position has been identified as having the potential for exposure to Hepatitis B. Access to training and a voluntary vaccination against Hepatitis B is provided.
