Key Responsibilities
- Patient Evaluation & Treatment Planning
- Conduct thorough assessments of patients referred by physicians.
- Develop individualized, goal-driven treatment plans and adjust them as needed.
- Collaborate with physicians to review and approve treatment strategies.
- Direct Patient Care
- Provide hands-on physical therapy to improve mobility, balance, strength, and endurance.
- Utilize therapeutic exercises, manual therapy, and modalities to facilitate recovery.
- Instruct patients on gait training, assistive device use, and fall prevention.
- Create and implement personalized home exercise programs.
- Interdisciplinary Collaboration
- Partner with nursing staff, physicians, and other rehabilitation professionals to ensure coordinated care.
- Educate patients' families and caregivers on treatment goals and progress.
- Participate in discharge planning to ensure continuity of care.
- Documentation & Compliance
- Maintain accurate, timely records of evaluations, treatment plans, and patient progress notes.
- Ensure adherence to Medicare and other regulatory standards.
- Record treatment charges in accordance with established billing guidelines.
- Team Leadership & Supervision
- Mentor and supervise Physical Therapist Assistants (PTAs) and therapy students.
- Lead in-service training sessions on advanced therapy techniques and best practices.
- Engage in facility meetings and performance improvement initiatives.
What We're Looking For
- Licensure: A current, valid state license as a Physical Therapist.
- Experience: Prior experience in a skilled nursing or rehabilitation setting is preferred.
- Clinical Expertise: Proficiency in diagnostic evaluation, treatment planning, and hands-on patient care.
- Communication: Excellent interpersonal skills with the ability to thrive in a multidisciplinary team.
