SUMMARY

Case Manager report to the Director of Social Services

Principal Duties and Responsibilities:

Under the general supervision of the Director of Social Services, Case Managers are expected to perform the following principal duties and responsibilities. 

  • Conduct initial intake, including but not limited to social service assessments, psychosocial assessments, independent living plans, and referrals for incoming clients who are referred from the shelter system.
  •  Conduct updates to psychosocial assessments and other evaluations as a basis for developing appropriate referrals and other interventions.
  •  Identify benefits and entitlements and connect clients to mental health, medical treatment, and substance use treatment facilities where applicable.
  •  Obtain identification, public benefits, and other applicable income sources for clients.
  •  Conduct weekly and bi-weekly meetings with clients to finalize their Individual Living Plans, assess their ongoing needs, promote harm reduction strategies, and facilitate referrals for necessary services.
  •  Document all efforts in providing services, interactions, and follow-up regarding resident involvement.
  •  Manage and track employment and housing placements of shelter residents.
  •  Maintain current and confidential client case records for the assigned caseload.
  •  Complete all required trainings in a timely manner.
  •  Teach residents how to advocate for themselves when seeking employment in the community.
  •  Facilitate clients in achieving their employment objectives through the identification and referral to pertinent services.
  •  Support the Program Director and Director of Social Services in achieving performance objectives, including resident rehousing placement goals.
  •  Collaborate with all Department of Homeless Shelter (DHS), Human Resources Administration (HRA), governmental bodies, and other relevant resources to advocate for and ensure the timely fulfillment of client housing needs.
  •  Facilitate workshops for residents on an as-needed basis, covering the housing search, application, and interview processes.
  •  Attend all mandatory meetings, including those pertaining to social services, team collaborations, and agency-wide initiatives.
  •  Ensure all mandatory agency and funder training modules are completed promptly.
  •  Perform other related duties as assigned.
Qualifications

Qualification:

Degree Requirement: Bachelors relevant to human services and at least one (1) year of proven experience in Human Service-related field or;

An associate degree with at least two (2) – three (3) years of proven experience in a Human Service-related field.

Bi-lingual preferred.

Familiarity with CARES, preferred.

Other Qualifications:

  •  Familiarity with NYC Shelter System with any population; Families with children, single adults, mental health, substance use, HIV/AIDS, or any other related field.
  •  The ideal candidate will demonstrate sensitivity, tact, and resourcefulness.
  •  A strong desire to assist clients in a nonjudgmental, empathetic, and compassionate manner is essential.
  • The capacity to effectively address cultural distinctions and a dedication to the importance of cultural competency are essential.
  • The ability to establish and maintain appropriate professional boundaries.
  • Demonstrates exceptional organizational, time management, and written communication abilities.
  • Experience working with DHS and other service providers, Preferred.
  • Experience providing case management services to families with children/Single Adults referred from DHS.

Physical Requirements:

Prolonged periods of sitting at a desk and working on a computer.

Must be able to lift to 25 pounds at times

Must be able to access and navigate each department at the organization's facilities.
