Job Overview
We are seeking a compassionate and experienced Hospice Case Manager to coordinate and deliver comprehensive end-of-life care for patients in various settings. The ideal candidate will possess an expertise in patient assessment, discharge planning, and interdisciplinary collaboration. Duties

• Conduct thorough patient assessments to determine care needs, including vital signs, physiological status, and psychosocial factors.
• Develop and implement individualized care plans in collaboration with healthcare teams, patients, and families.
• Coordinate services across inpatient, outpatient, clinic, and post-acute settings to ensure seamless transitions of care.
• Manage discharge planning processes, including documentation and communication with relevant providers and caregivers.
• Monitor patient progress through diagnostic evaluations, medical records review, and ongoing assessments.
• Facilitate utilization management by evaluating the appropriateness of services and resource utilization.
• Maintain compliance with HIPAA regulations to protect patient confidentiality and privacy.
• Collaborate with multidisciplinary teams including nursing, social work, physicians, and therapists to optimize patient outcomes.
• Educate patients and families on disease processes, symptom management, and available resources related to hospice care.
• Document all interactions accurately within medical records systems and ensure timely updates.

Requirements

• Graduate of an accredited school of medicine, social work, nursing or related medical occupation.
• At least 3 years of Case management experience.
• Previous experience with coordinating community resources for post-acute hospitalization care preferred.
• Active Oregon RN License
