About Us
Heartland Center for Autism is a specialized residential treatment program serving youth ages 6–17 with Autism Spectrum Disorder (ASD) and Intellectual & Developmental Disabilities (IDD). We provide individualized, evidence-based care within a structured, compassionate, and trauma-informed environment.
We are currently seeking a Case Manager to join our Leadership Team at our Okemos residential campus, an impactful role for a individual who wants to combine coordination, clinical work, and meaningful relationships with youth and families.
Position Overview
As a Case Manager at Heartland, you'll play a central role in guiding each resident's journey—from admission through discharge—ensuring services are coordinated, compliant, and deeply person-centered.
This role blends clinical leadership and direct therapeutic impact. In addition to managing referrals, IPOS development, documentation, and family collaboration, the Case Manager provides up to two (2) hours of individualized psychosocial therapy daily, supporting residents' emotional regulation, communication skills, and overall well-being.

What You'll Do
- Coordinate admissions, intakes, and psychosocial assessments
- Manage referrals and collaborate with clinical leadership on program fit and placement decisions
- Lead development, implementation, and review of Individual Plans of Service (IPOS)
- Provide up to 2 hours per day of individualized psychosocial therapy using evidence-based approaches
- Maintain accurate, timely, and compliant documentation aligned with MDHHS and CARF standards
- Coordinate provider appointments, family trainings, and interdisciplinary collaboration
- Lead discharge planning and ensure smooth transitions to community or family settings
- Serve as a primary liaison between families, residents, staff, and external providers
- Participate in interdisciplinary meetings, audits, and program development initiatives
What We're Looking For
- Bachelor's degree required in Social Work, Psychology, Human Services, or related field
- Master's degree in Social Work (MSW) preferred
- Michigan state licensure preferred
- 2+ years of experience in clinical, case management, or residential treatment settings preferred
- Strong clinical judgment, documentation, and communication skills
- Familiarity with trauma-informed care, MDHHS, and CARF standards is a plus
Schedule
- Full-time: Monday–Friday, 9:00 AM – 5:00 PM, in person
- Occasional evening or weekend flexibility for admissions, discharges, or family trainings
