Community Based service Provider

Duties

• Provide direct support and assistance to clients in their own homes or community settings, ensuring their safety and well-being.
• Develop personalized care plans tailored to each individual's needs, preferences, and goals.
• Facilitate access to community resources such as healthcare, education, employment, and social activities.
• Assist clients with daily living activities including mobility, medication management, and household tasks.
• Monitor client progress and document interactions accurately to ensure consistent quality of care.
• Collaborate with families, caregivers, and multidisciplinary teams to coordinate services effectively.
• Promote independence by empowering clients through skill-building activities and motivational support.

Experience

• Prior experience working in community outreach, social services, or related fields is highly desirable.
• Strong interpersonal skills with the ability to build trusting relationships with diverse populations.
• Excellent communication skills—both verbal and written—to effectively document and report client progress.
• Ability to work independently while managing multiple priorities in a fast-paced environment.
• Knowledge of local community resources and social service programs is a plus.
• Demonstrated commitment to client confidentiality and ethical standards.
• Flexibility to adapt to changing needs and participate in ongoing training or professional development.
