About the Role

The Home Health Aide (HHA/CNA) at Elmwood Healthcare - Home Health (Elmwood Home Care ) will provide essential personal care and support services to patients in their homes, assisting them with daily living activities and ensuring their comfort and well-being. You will work under the supervision of a registered nurse or healthcare professional to deliver care that meets the individual needs of each patient.

Monday through Friday; 8:00 A.M. to 4:30 P.M.; On-Call Rotation

Key Responsibilities:

  • Personal Care: Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and eating. Provide support with mobility, transfers, and positioning.
  • Companionship and Emotional Support: Offer companionship and emotional support to clients, helping to alleviate feelings of loneliness and promoting a positive, supportive environment.
  • Medication Assistance: Remind clients to take prescribed medications as directed. Monitor for any changes in health and report concerns to the supervising nurse or healthcare professional.
  • Household Assistance: Perform light housekeeping tasks such as cleaning, meal preparation, and laundry to maintain a safe and comfortable living environment for clients.
  • Health Monitoring: Observe and report any changes in clients' physical or mental health to the supervising nurse or healthcare professional. Assist with measuring and recording vital signs if required.
  • Documentation: Maintain accurate and up-to-date records of the care provided, including any changes in clients' conditions, and ensure proper documentation is completed in accordance with company policies.
  • Safety and Compliance: Adhere to all safety and infection control protocols to ensure a safe environment for both clients and staff. Follow all relevant regulations and best practices in home healthcare.
