Youth Development Services include, but are not limited to, providing life skills training to youth residing

in licensed foster care or group/residential placements, facilitating the completion of Teen Plans, Education

Plans and assessments in accordance with Statute. In addition, Youth Development Services work to

provide opportunities for youth to enhance their lives by participation in extracurricular, educational and

mentorship activities. Youth Development Services provide support, suggestions and tips to caregivers and

providers to insure stability for the youth in the home.

IL Transition & Aftercare Services are responsible for ensuring that eligible young adults have the

assistance and opportunity to earn a high school diploma/GED, obtain employment skills, health care, life

skills, mentors and assistance entering post-secondary training and education programs. In addition, IL

Transition & Aftercare Services administers financial assistance to eligible young adults receiving Road to

Independence, Transition and/or Aftercare. Independent Living Services support youth and young adults'

efforts to have a quality of life appropriate for their age and to assume personal responsibility for becoming

self-sufficient adults.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be

assigned.

  1. Provide monthly supervision to Youth Development Specialists and Transition Specialists

  2. Monitor and report on contractual outcomes related to the completion of Teen Plans, Education

Plans, Assessments and Life Skills plans, and trainings.

  1. Coordinate and insure that IL service delivery is community based allowing for experiential

learning opportunities for teens and young adults

  1. Facilitate and update all 17, 17 ½ and 17 ¾ IL Transitional Service Plans/staffing.

  2. Take lead on advocating for needed services for 17 year olds preparing to transition from care at

the Permanency staffing and/or 17, 17 ½ and 17 ¾ staffings and provide information for the

required Judicial Review

  1. Provide targeted IL Transition & Aftercare services for year olds who are receiving services

through Aftercare or RTI programs

  1. Coordinate and/or conduct hands-on training for skills necessary to live independently; training

will consist of at a minimum budgeting, nutrition,family planning, education planning, job

training, obtaining & maintain housing.

  1. -Coordinate and/or conduct in-services for staff and foster parents addressing the issues and needs

of youth aging out of foster care

  1. Document services in FSFN within 48 hours of service delivery

  2. Monitor and report on staff compliance related to case related documentation including but not

limited to, email, phone and case note documentation

  1. Complete and review all applications for Transitional Support, Aftercare and RTI.

  2. Monitor and insure the timeliness and accuracy of all applications and renewals.

  3. Monitor and implement programming to reduce the incidence of teen runaway behavior

  4. Attend court as required..

  5. In the absence of the IL Director, ensure proper documentation for all training, budgets and

services provided.

  1. Assist Director of Independent Living Services in community activities and presentations

Master's degree (M. A.) in applicable Human Services or related field; or a Bachelor's degree (B.A.)

from four year college or university and three years related experience and/or training; or equivalent

combination of education and experience.
