Position Overview

The Case Manager at The Free Medical Clinic is a key part of our interdisciplinary care team, committed to delivering holistic, patient-centered care to underserved populations. This role involves direct patient interaction, care coordination, and collaboration with medical providers to address the social, emotional, and financial challenges that impact health outcomes.

Key Responsibilities



Patient Assessment & Care Planning
: Conduct psychosocial assessments to evaluate patients' emotional, social, and financial needs. Develop individualized care plans in collaboration with providers and clinic staff.


• 
Care Coordination, Discharge & Follow-Up
: Support patients by identifying barriers to care and connecting them with appropriate resources. Coordinate discharge plans and ensure continuity of care through referral tracking, appointment scheduling, and proactive follow-up.


• 
Resource Coordination
: Connect patients with vital community resources, including food assistance, housing programs, behavioral health services, transportation, and financial aid. Assist patients with navigating healthcare and insurance systems as needed. Build and maintain relationships with community agencies that serve like populations.


• Documentation

: Maintain timely, accurate, and confidential documentation of assessments, interventions, and follow-ups in accordance with ethical and legal standards.


• 
Team Collaboration
: Participate in regular interdisciplinary team meetings to coordinate care and contribute to a holistic approach to treatment and patient support.

Qualifications



Education:
Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited institution.


• 
Licensure:
 LBSW, LMSW, or LCSW preferred.


• Experience: Prior experience in healthcare, nonprofit, or community-based settings is highly desirable; at least two years of experience with vulnerable populations is preferred. Familiarity with medical terminology and social determinants of health is a plus.


• 
Skills & Attributes:

- Knowledge of local and regional community resources

- Experience with EHR systems preferred

Working Conditions



Schedule:
 Monday– Thursday 7:30 am – 4:30 pm and Friday 7:30 am – 12 pm.

 Located in an urban community clinic setting in downtown Columbia, SC.
