Job Overview
We are seeking a dedicated and knowledgeable MDS Coordinator to join our healthcare team. The ideal candidate will be responsible for overseeing the assessment and documentation processes related to the Minimum Data Set (MDS) in compliance with federal and state regulations. This role requires a strong understanding of clinical documentation, acute care, and utilization management to ensure optimal patient care and accurate reporting.

Duties

  • Conduct comprehensive assessments of patients using the MDS tool to ensure accurate documentation of their clinical status.
  • Collaborate with interdisciplinary teams to gather necessary clinical information for MDS submissions.
  • Review clinical documentation for accuracy and completeness, implementing improvements as needed.
  • Ensure compliance with all regulatory requirements related to MDS submissions and reporting.
  • Provide education and training to staff on MDS processes, documentation standards, and best practices.
  • Utilize electronic health record systems, such as Epic, for efficient data management and reporting.
  • Participate in quality improvement initiatives aimed at enhancing patient care outcomes.
  • Maintain up-to-date knowledge of changes in regulations affecting MDS processes.

Experience
Candidates should possess the following qualifications:

  • Must have PCC experience
  • Must have, at a minimum, two (2) years of experience as a supervisor in a hospital,
  • nursing care facility, or other related health care facility.
  • Must have, at a minimum, six (6) months training experience in rehabilitative and restorative nursing practices.
  • Strong knowledge of clinical documentation improvement strategies.
  • Familiarity with MDS processes and requirements is highly desirable.
  • Understanding of anatomy and physiology as it relates to patient assessments.
  • Experience in utilization management or utilization review is a plus.
  • Proficiency in using electronic health record systems, particularly Epic, is preferred. Must be able to move intermittently throughout the workday. Speak and write the English language in an understandable manner.
  • Cope with the mental and emotional stress of the position.
  • Function independently, and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.
  • Must have patience, tact, a cheerful disposition and enthusiasm.
  • Meet the general health requirements set forth by the policies of this facility.
  • Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and at times hostile people within the facility.
  • Ability to bend, stoop, sit, stand and reach.
  • Ability to push, pull, move, and/or lift items weighing 25 pounds or less.
