Duties

  • Conduct thorough patient assessments, including vital signs, physical examinations, and detailed medical histories across various populations such as children, seniors, and individuals with disabilities.
  • Manage acute and chronic conditions by developing personalized treatment plans that incorporate medication administration, IV infusion therapy, catheterization, and wound care techniques like suturing.
  • Perform specialized procedures including Botulinum toxin treatments, dermal fillers, esthetic laser treatments, spinal taps, and sonography to enhance patient outcomes.
  • Collaborate with physicians and specialists in areas such as gastroenterology, infectious disease care, hospice & palliative medicine, and Level I trauma center protocols to ensure optimal patient management.
  • Utilize electronic health record (EHR) systems like Epic, Cerner, eClinicalWorks, and Athenahealth for accurate medical documentation, CPT coding, ICD-10/ICD-9 coding, and compliance with HIPAA regulations.
  • Provide patient education on health coaching topics such as nutrition, medication adherence, disease prevention (including Workers' compensation law), and post-discharge planning.
  • Participate in clinical research projects or quality improvement initiatives to advance evidence-based practices within the organization.

Requirements

  • Valid Nurse Practitioner license with current certification to practice independently in your state.
  • Demonstrated experience in diverse clinical settings including hospital medicine, emergency medicine, ICU/CCU/PICU units, nursing homes, assisted living facilities or home care environments.
  • Proficiency with EMR/EHR systems such as Epic or Athenahealth; telehealth experience is highly desirable.
  • Strong knowledge of physiology, anatomy, pharmacology (including psychopharmacology), and diagnostic evaluation techniques.
  • Experience administering injections (including Botox or dermal fillers), performing venipuncture/venipuncture/catheter care and sterile processing techniques.
  • Critical care experience from Level II or Level I trauma centers or ICU settings; familiarity with ventilator management and airway management protocols is preferred.
  • Ability to work effectively with diverse populations including pediatrics (Toddler & Neonatal care), geriatrics (Memory & Alzheimer's care), behavioral health patients or individuals with developmental disabilities.
  • Excellent communication skills for triage assessments, discharge planning, case management and working within multidisciplinary teams focused on patient-centered care. Job Type: Full-time

Pay: $105, $130,000.00 per year
