$100,000 – $140,000 annually
Position Overview:
This role involves traveling to patient residences within a typical radius of up to 45 miles from your home base, with occasional trips of up to 60 miles once or twice per month.
At HomeMD, our mobile Nurse Practitioners deliver essential medical care to older adults directly in their residences—whether in assisted living or other home-based settings. By bringing care onsite, we eliminate transportation barriers and allow seniors to receive timely, personalized healthcare in a familiar environment.
Core Duties and ResponsibilitiesRoutine Medical Evaluations
- Conduct physical assessments, routine check-ups, and health screenings
- Monitor ongoing conditions such as blood pressure, glucose levels, and cholesterol
- Identify changes in patient status and adjust care plans accordingly
Management of Chronic Conditions
- Provide ongoing care for long-term illnesses including diabetes, hypertension, arthritis, heart disease, and dementia
- Adjust treatment plans and medications to support optimal health outcomes
Medication Management
- Review current medications for effectiveness, interactions, and side effects
- Collaborate with pharmacists and prescribing providers on medication updates
- Educate patients and caregivers on proper medication use
Palliative and End-of-Life Support
- Deliver compassionate, patient-centered care for individuals with terminal or advanced conditions
- Focus on comfort, symptom control, and quality of life
Preventive Care Services
- Administer vaccinations such as influenza and pneumonia
- Perform preventive screenings and provide education on injury and illness prevention
Telehealth Support for Acute Needs (when applicable)
- Address urgent medical concerns including infections, falls, or respiratory symptoms
- Part-time providers have access to a clinical support team for urgent situations
Care Coordination
- Collaborate with physicians, specialists, caregivers, and facility staff
- Contribute to comprehensive and individualized care planning
Patient and Caregiver Education
- Provide education on nutrition, mobility, mental wellness, and fall prevention
- Support families and facility staff with health-related guidance
Services Commonly Provided by Mobile NPs
- Primary Care: Diagnosis, treatment, and long-term health maintenance
- Behavioral Health Support: Assistance with anxiety, depression, and dementia-related symptoms, with escalation to BHI teams when needed
- Fall Risk Assessments: Identification of risk factors and preventive recommendations
- Wound & Foot Care: Treatment and monitoring of wounds, pressure injuries, and podiatric concerns, with access to advanced wound care teams
- Screening & Monitoring: Ordering labs, vision/hearing exams, and other preventive tests
How the Role Is Supported
- Appointment scheduling is handled by medical assistants
- Remote support staff assist with administrative tasks and order management
Qualifications
- Education: Master's or Doctorate in Nursing
- Certification: Board-certified Nurse Practitioner
- Licensure: Active Michigan NP license
- Experience: Geriatrics, chronic care, urgent care, or palliative care experience preferred
