Community Support Advocate

Overview
We are seeking a compassionate and dedicated Direct Support Professional to join our team. In this role, you will provide essential support to individuals with developmental disabilities, assisting them in achieving their personal goals and enhancing their quality of life. You will work closely with clients in various settings, including home health environments, assisted living facilities, and community programs. Your commitment to care and support will make a significant difference in the lives of those you serve.

Responsibilities

• Assist clients with daily living activities, including personal hygiene, meal preparation, and medication administration.
• Develop and implement individualized care plans tailored to each client's needs and preferences.
• Monitor and document clients' vital signs and overall health status while ensuring compliance with HIPAA regulations.
• Provide companionship and emotional support to enhance clients' well-being.
• Support clients in engaging with their communities through social activities and skill-building opportunities.
• Collaborate with healthcare professionals to ensure comprehensive care for clients with dementia or other health conditions.
• Maintain a clean and safe environment for clients, adhering to all safety protocols.

Requirements

• Previous experience in senior care, home health, or working with individuals with developmental disabilities is preferred.
• Knowledge of medication administration practices and ability to manage care plans effectively.
• Familiarity with dementia care techniques and the ability to provide compassionate support for clients experiencing cognitive decline.
• Strong communication skills and the ability to build rapport with clients, families, and team members.
• Ability to perform basic health assessments, including taking vital signs.
• Understanding of HIPAA regulations and commitment to maintaining client confidentiality.
• A positive attitude, patience, and a genuine desire to help others improve their quality of life.
