This position rotates on-call responsibilities with the other Community Resident Care Coordinator and must be prepared to provide staffing coverage in the event of staff shortages or call-outs.

Join our dedicated team as a Resident Care Coordinator and become a vital part of providing compassionate, comprehensive care to our residents. In this role, you will coordinate and oversee personalized care plans, assist residents with Activities of Daily Living (ADLs), and ensure their well-being in a safe, respectful environment. Responsibilities

  • Develop, implement, and regularly update individualized care plans tailored to each resident's needs, including those requiring Alzheimer's, dementia, or memory care.
  • Assist residents with ADLs such as bathing, dressing, grooming, mobility, and meal preparation, ensuring dignity and independence are maintained.
  • Monitor residents' vital signs and medication administration in accordance with medical directives and HIPAA regulations.
  • Coordinate with nursing staff, social workers, and families to ensure seamless case management and communication regarding resident health status.
  • Utilize EMR (Electronic Medical Record) systems to document care activities accurately and maintain comprehensive resident records.
  • Support residents during activities that promote social engagement, behavioral management, and emotional well-being.
  • Conduct safety checks including Hoyer lifts, resident transfers using assistive devices, and heavy lifting when necessary to prevent injury.
  • Manage conflict resolution effectively while fostering a positive environment for residents and staff alike.
  • Assist with light housekeeping duties such as cleaning resident areas to uphold hygiene standards.
  • Drive residents when needed for appointments or outings while adhering to all safety protocols.

Qualifications

  • Prior experience in assisted living facilities or nursing homes is highly preferred; experience working with people with developmental disabilities or autism is a plus.
  • Knowledge of Alzheimer's care, memory care strategies, dementia care techniques, and hospice care principles.
  • Strong understanding of medical terminology, medication administration procedures, vital signs monitoring, and HIPAA compliance.
  • Excellent communication skills for collaborating with healthcare teams, families, and residents' support networks.
  • Ability to perform caregiving tasks including assisting with ADLs, lifting residents safely using Hoyer lifts or other assistive devices, and managing behavior issues compassionately.
  • Valid driver's license preferred for transportation duties; willingness to undergo first aid certification is advantageous.

Job Type: Full-time

Pay: $24.00 per hour

Expected hours: 32 – 40 per week
