Summary
The Case Manager provides high-quality, client-centered services for individuals enrolled across A Safe Haven programs, including DFSS Interim Housing, No Place to Stay (NPTS), Veterans Housing, and Recovery Homes. Each Case Manager will be assigned a "home program" for primary placement; however, the expectation is that all Case Managers operate inter-programmatically to provide unified, unsiloed, and comprehensive care. This approach ensures that every client can receive consistent support across programs and that the organization maximizes team capacity, continuity, and equity in service delivery.
Key Responsibilities
Client Assessment & Engagement
- Conduct comprehensive intake and biopsychosocial assessments for new clients in assigned program(s).
- Develop individualized service plans that integrate housing, recovery, employment, health, and justice-related needs.
- Build therapeutic, professional relationships with clients using trauma-informed, strengths-based practices.
Case Management & Coordination
- Provide regular one-on-one case management sessions, adjusting service plans as needs evolve.
- Collaborate across programs to ensure continuity of care and equitable access to services, regardless of a client's assigned housing program.
- Facilitate connections to in-house resources (employment, education, clinical services, recovery supports) and community partners.
- For NPTS clients, coordinate with the Office of the Chief Judge and electronic monitoring officials to ensure compliance and support successful re-entry.
Documentation & Compliance
- Maintain timely, accurate case notes, service plans, and progress updates in accordance with program, funder, and regulatory standards.
- Track and report client outcomes across programs, ensuring consistency in data entry and compliance.
- Adhere to confidentiality and ethical standards of practice.
Team Collaboration & Inter-Program Work
- Participate in interdisciplinary case reviews to promote inter-programmatic planning and accountability.
- Provide coverage and support across DFSS Interim, NPTS, Veterans, and Recovery Home programs as needed.
- Contribute to the development of best practices that reduce silos and strengthen A Safe Haven's integrated model of care.
Advocacy & Support
- Advocate for client access to services and fair treatment across systems of care.
- Provide crisis intervention, de-escalation, and conflict resolution support as needed.
- Encourage client accountability, empowerment, and engagement in recovery and stability goals.
Qualifications
Required:
- Master's degree in social work (MSW), OR Bachelor's degree in Social Work with LSW licensure.
- For Master's applicants: LSW or LCSW strongly preferred.
- Minimum 1–2 years of experience providing case management, counseling, or clinical services in housing, recovery, or justice-involved settings.
- Strong knowledge of trauma-informed care, harm reduction, and evidence-based recovery and re-entry practices.
- Excellent communication, collaboration, and problem-solving skills.
Preferred:
- Experience with justice-involved populations, veterans, or individuals in recovery.
- Familiarity with DFSS, IDHS, and Office of the Chief Judge program requirements.
- Bilingual skills (English/Spanish or other languages relevant to client population).
