Hospice Nurse Practitioner

Hospice-Nurse Practitioner-St. Cloud, MN

We are seeking a compassionate and experienced Advanced Practice Provider to join our dedicated Hospice Care team. The APP will work collaboratively with the hospice interdisciplinary team to deliver end-of-life care to patients. 

• Part time, .6 FTE, 3 days per week M-F 8-5 with shared call responsibility.
• Provide direct patient care, including comprehensive assessments, symptom management, and treatment planning
• Serve as a clinical resource for hospice staff and external care teams

Ideal Candidates would: 

• Must be BE/BC and able to obtain a MN License.
• Experience in Hospice or end of life care preferred
• Current licensure as a Nurse Practitioner (NP) in the state of Minnesota
• Valid driver's license and reliable transportation

Pay and Benefits

• Starting pay begins at $109,990 per year; exact wage determined by years of related experience 
• Salary range:  $109,990 - $139,880 per year 
• Salary and salary range are based on a 1.0 FTE, reduced FTE will result in a prorated offer rate
