Position Summary:
This position entails managing a caseload of families involved with Children and Youth Services (CYS) residing in the Family Management Center Shelter. The participating families face barriers that require intensive support services to secure and maintain housing, achieve financial stability, and keep their families intact.
Primary Duties and Responsibilities:
- Manage a caseload of 11 homeless economically disadvantaged individuals and families requiring case management and supportive services in the Shelter Facility.
- Conduct Intakes, develops and updates stability plans upon entrance into the Shelter in collaboration with CYS and already established CYS goals and self-sufficiency plans.
- Coordinate on-going collaborations with CYS Case Workers and Administrators.
- Conduct formal inspections of client units and provide housekeeping education and support as needed.
- Develop and update individual service plans and self-sufficiency goals for each client.
Maintain at a minimum weekly contact with clients to assure client compliance with goal plans and shelter rules and regulations.
Document ongoing work with each client including progress notes, service plan development and updates to identifying and alleviate systemic barriers to client self-sufficiency.
Develop a network of social services, refer clients to appropriate resources, identify and coordinate on-going community linkages and provide advocacy and support.
- Assist in the instruction of basic life skills, including home maintenance, comparative shopping, budgeting, laundry, health/hygiene, nutrition, parenting, self-esteem, communication, and other topics as needed/requested.
Monitor and support client progress throughout and conduct 6 months' post-exit follow-up.
Attend CYS-mandated meetings and trainings
Complete reports as required by funding sources, monitoring agencies, and the
COO and CEO.
Participate in required staff meetings and client review meetings.
Other related duties as required.
Position Objectives:
- Provide excellent supportive services to clients that will result in meeting individual client goal plans toward self-sufficiency.
- Develop and utilize a broad network of resources to best service clients.
- Display a professional role model for clients.
- Maintain accurate, up-to-date client files.
Qualifications:
This position requires a minimum of a Bachelor's Degree in Human Services, Social Work, Psychology or a related field from an accredited college or university. A Master's Degree is preferable. Requires a minimum combination of education and experience that fulfills the knowledge and experience base required for this position.
The person in this position must demonstrate outstanding organizational skills and the ability to manage multiple priorities and concerns. This position requires an individual with outstanding clinical skills and the ability to work well with people from different cultural and socio-economic backgrounds. The individual must have excellent oral and written communication skills, combined with strong interpersonal skills.
Physical Demands:
The physical demands described in this section are representative of those that must be met by an employee to successfully perform the essential functions of this position. While performing the duties described herein, the employee is regularly required to talk, hear, stand, walk, maneuver stairs, sit, stoop, kneel, crouch, and crawl. The employee must occasionally lift and/or move up to 15 pounds. Specific vision capabilities required by this position include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus.

Pay: $48, $56,000.00 per year
