partner facility or provider. All care is based solely on patient needs.
- Our multidisciplinary team—including physicians, nurse practitioners, social workers,
and chaplains—works together to ensure that every patient receives thoughtful,
individualized, clinically appropriate care.
Position Overview
- The Supportive Care Specialist Nurse Practitioner (NP) plays a key role in delivering
high-quality palliative care services across acute and long-term care settings. This
position focuses on symptom management, care coordination, goals-of-care
conversations, and supporting patients and families through complex medical decisions.
- This role emphasizes clinical excellence, ethical practice, and patient-centered care. No
duties involve soliciting, influencing, or rewarding referrals or business tied to
federally reimbursed services. All activities must comply with the Anti-Kickback
Statute (AKS) and Stark Law.
Primary Responsibilities
Duties include but are not limited to:
- All responsibilities must be performed solely for patient care purposes and may not be
influenced by or tied to the volume or value of referrals.
- Conduct daily rounding in acute care and LTAC facilities, delivering compassionate and
clinically appropriate palliative care.
- Assess, diagnose, and manage symptoms such as pain, dyspnea, nausea, and anxiety to
promote comfort and quality of life.
- Collaborate with attending physicians, facility staff, and SCS clinicians to coordinate care
and ensure continuity across settings.
- Facilitate goals-of-care discussions and assist with advance care planning.
- Document all patient encounters thoroughly, accurately, and in compliance with the
Electronic Health Record (EHR) requirements.
- Provide education to patients, families, and healthcare teams regarding palliative care
principles.
- Participate in interdisciplinary team meetings and contribute to comprehensive care
planning.
- Demonstrate professionalism, empathy, and strong clinical judgment in all patient
interactions.
- Maintain clear, compassionate communication with patients and families as they navigate
emotionally challenging situations.
- Uphold all legal, ethical, and regulatory standards, including full compliance with AKS,
Stark, HIPAA, and organizational policies.
Qualifications
- Active Nurse Practitioner (NP) license in the state of practice.
- Master's or Doctoral degree in Nursing from an accredited institution.
- Active or eligible DEA registration.
- Board Certification in Family Practice, Adult-Gerontology, or Acute Care (Palliative or
Hospice certification preferred).
- Minimum of 2 years of clinical experience in palliative care, acute care, internal
medicine, or geriatrics preferred.
- Strong communication skills and the ability to support patients and families through
complex situations.
- Commitment to patient autonomy, dignity, and high-quality, ethical care.
Compensation and Benefits
- Compensation is established in advance, consistent with fair market value (FMV),
and is not dependent—directly or indirectly—on the volume or value of referrals or
other federally reimbursable services.
- Salary range: $145,000 – $165,000 per year (based on experience and FMV standards)
- Additional professional development–based or quality-based opportunities may be
available, but none are tied to referral volume or federally reimbursed services

Schedule
- Full-Time, Monday–Friday (Day Shift)
- Flexible rounding schedule based on patient census and clinical need
- Limited weekend or after-hours phone coverage as needed
