The Social Worker supports the overall wellbeing of residents and their families by providing counseling, advocacy, and care planning. This position plays a key role in promoting a positive quality of life for residents at Sunshine Meadows through emotional support, resource coordination, and collaboration with the interdisciplinary team.
Key Responsibilities
Provide social services to meet the emotional, social, and psychological needs of residents. Assist residents and families with adjustment to facility life, including admission, transition, and discharge planning. Conduct assessments, maintain required documentation, and participate in care plan development. Serve as an advocate for residents, ensuring their rights and preferences are respected. Provide counseling and support to residents and families during times of transition, loss, or crisis. Facilitate family conferences, support groups, and communication between residents, families, and staff. Coordinate community resources and referrals as needed (e.g., mental health services, hospice, community programs). Maintain compliance with all federal, state, and facility regulations regarding social services. Collaborate with nursing, activities, and other departments to support resident-centered care. Qualifications
Bachelor's degree in Social Work (BSW) required. Current Kansas Social Work license (LSCSW or LMSW) in good standing. Previous experience in long-term care, healthcare, or geriatrics preferred. Strong communication, advocacy, and problem-solving skills. Ability to work effectively with residents, families, staff, and community partners.
