Job Overview
As a Case Manager, you will play a pivotal role in guiding individuals through challenging situations and helping them achieve their personal goals. You will utilize your expertise in crisis intervention and behavioral health to provide comprehensive support to clients facing various life challenges, including addiction and child welfare issues. Your passion for mentoring and workforce development will empower clients to build a brighter future.

What you'll do

  • Conduct thorough assessments to identify client needs and develop tailored service plans.
  • Provide crisis management and intervention strategies to support clients in high-stress situations.
  • Facilitate addiction counseling sessions, utilizing motivational interviewing techniques to encourage positive change.
  • Collaborate with community resources, including Section 8 housing assistance and Medicare services, to connect clients with essential support.
  • Supervise and mentor junior staff members, fostering a collaborative environment focused on client success.
  • Advocate for clients within the child welfare system, ensuring their rights and needs are prioritized.
  • Monitor client progress and adjust service plans as necessary to ensure effective outcomes.
  • Engage in ongoing professional development to stay current with best practices in social work and behavioral health.

Basic qualifications
● Must be at least 18 years of age.
● Must have a high school diploma or GED equivalent.
● Must have at least one year of experience working with behavioral health populations.
● Must be experienced working with a diverse population including persons impacted upon by mental illness.
● Must have criminal background clearance.
● Must have a completed TB test, CPR/First Aid certificate, and health screening

Preferred qualifications

  • Strong background in addiction counseling and behavioral health practices.
  • Proven supervisory experience with a focus on team development.
  • Familiarity with childcare regulations and child welfare systems.
  • Knowledge of workforce development strategies that empower individuals towards self-sufficiency.

● Ensures all intake documents are completed, signed, and dated for each client.
● Ensures client confidentiality and safety while maintaining appropriate boundaries with clients.
● Verifies that the Program Director has basic consumer information, medical history, and all signed waivers and releases on new clients.
● Protects resident confidentiality, as required by State and Federal Privacy Laws.
● Completes incident reports.
● Reviews and completes the admission checklist for each new client.

● Assists, teaches, and or supervises residents with program schedule activities and community rules.
● Observes client participation in a casual setting, such as group sessions, and outings to evaluate mental and physical status and progress.
● Confers with other case managers and the Program Director to provide input on client status and progress.
● Leads discussion groups and community meetings, as well as providing group instruction on mental health and related topics.
● Participates in the development, revision, and implementation of individualized treatment plans and discharge/aftercare plans.
● Inspects clients' belongings for contraband and other unauthorized items as necessary.

Job Type: Full-time

Pay: $ $38.00 per hour
