Summary
El Nido Family Centers is seeking a full-time, Lead Case Manager to join our team. Annual Salary: $60,000 based on a 37.5 hrs/weekly
The Lead Case Manager will provide intake/assessment and comprehensive case management services to individual clients and their families and will also provide guidance to other case managers and college corner guides to ensure best practices are followed and client services and outcomes are optimized. This position requires in person work daily.
Essential Duties
- Conduct intakes, as needed with families and individual customers.
- Conducts pre-assessments, multi-benefit screening and comprehensive case management services with families/customers.
- Maintains frequent contact with customers through appointments at the FSC and telephone conversations to follow through with programs, services and provide ongoing support.
- Ensures that assigned cases to case managers are followed up in a timely manner.
- Provides guidance and support to case managers, ensuring quality of service delivery.
- Supports with the oversight of intake calendar in consultation with Program Manager.
- Refer, connect and link families/customers to FSC programs and activities, including DPSS, employment support, financial coaching, housing stability Services, LAUSD PSA counselor and other El Nido services, and community resources to assist with meeting goals of improving Family Income, Financial Stability and Academic Achievement
- Coordinate pre-employment and employment supportive services, with identified employment partners for clients to receive assistance/support with job search, employment applications, resumes, interview process and job readiness workshops.
- Participate in the development of individual college plans with families under the Parent Promise component with LAUSD PSA counselor and in collaboration with College Corner Guides
- Responsible for identifying services that will support program outcomes for adults and youth.
- Assists with the training and mentoring of interns, college corner guides, case managers on best practices in trauma informed care, case management and client advocacy.
- Supports Program Manager with College Corner strategic planning and enhancements.
- Monitors, tracks and reports on College Corner services, outcomes and contract compliance.
- Reinforces and maintains program policies and procedures.
- Conducts quality assurance reviews of ICM files, services delivery and program processes in consultation with Program Manager.
- Provides feedback to Program Manager on any noted performance issues and ensures compliance with documentation and reporting requirements.
- Provides some administrative support of the team, which includes coverage for the program manager when not available.
- Enters follow up services and progress notes in Salesforce data entry system.
- Tracks outcomes and submits monthly outcome log on a monthly basis to FSC Program Manager to monitor progress towards expected outcome goals.
- Serves as an advocate to help families/customers access and secure identified services.
- Participates in special events year-round. Special events include but are not limited to VITA Tax Preparation, Outreach Events, Backpack, Toy and Food Distribution events. Also, participates in events and projects assigned by CIFD/LA City.
- Ensures program compliance with performance metrics, funding requirements and agency policies.
- Meet and maintain caseload productivity requirements to meet
- Works with identified collaborative internal and external partners.
- Initiates crisis intervention when crisis related incidents occur with clients and supports other staff with crisis intervention efforts and ensures that proper response protocols are followed.
- Consults with Program Manager on crisis related matters.
- Assists Program Manager, Director and FSC staff with special projects and events that may require flexibility in work schedule.
Requirements
- Bachelor's degree in social work, psychology, child development or a related field
- 2-3 years in relevant case management, youth or social service experience
- Strong communication and interpersonal skills
- Strong organizational and time management skills to manage complex caseloads, prioritize tasks and maintain accurate records.
- Effective planning and problem-solving skills
- Must be able to work with diverse populations.
- Proficient in MS word and Excel; accurate math skills; tying 45-50 wpm.
- Ability to work some evenings and Saturdays based on program needs
- Valid driver's license and reliable transportation required, satisfactory driving record and proof of insurance.
