In this role, you’ll work closely with motivated patients in a modern outpatient setting, providing thoughtful evaluations, personalized treatment plans, and hands-on care that helps people move better, feel better, and return to the activities they love. You’ll be supported by a collaborative team that values growth, accountability, and pride in the work we do.

*What You’ll Do*
* Perform comprehensive physical therapy evaluations and develop individualized plans of care based on patient goals, movement quality, and evidence-based practice
* Deliver hands-on treatment sessions focused on restoring mobility, reducing pain, and improving function
* Utilize a blend of manual therapy, therapeutic exercise, neuromuscular re-education, and patient education to support long-term outcomes
* Monitor patient progress and adjust treatment plans thoughtfully to ensure continued improvement
* Educate patients on injury prevention, movement strategies, and lifestyle habits that support long-term wellness
* Document evaluations, treatments, and progress notes clearly and efficiently in our EMR system
* Collaborate with team members to uphold Evolve’s standards for clinical care, communication, and patient experience
* Contribute positively to clinic culture by showing up engaged, professional, and supportive of both patients and teammates
*What We’re Looking For*
* Licensed Physical Therapist (or license-eligible) in the state of Connecticut
* Strong foundation in anatomy, physiology, and orthopedic rehabilitation
* Passion for outpatient care and building meaningful patient relationships
* Confidence delivering hands-on treatment and coaching movement with intention
* Excellent communication skills and a warm, approachable presence
* Comfort with EMR systems and clear clinical documentation
* A team-first mindset with a desire to grow clinically and professionally

Job Types: Full-time, Part-time
Pay: $84,000.00 - $115,000.00 per year
Expected hours: 40 per week
